Fast Track To Home

burlington-still018Planning a safe discharge is paramount to every patient’s successful transition back to their prior living setting. The Fast Track to Home Program focuses upon the philosophy that discharge planning begins at admission.

Seventy-two hours after admission the interdisciplinary team meets with the patient to discuss initial assessments, the projected discharge date and any questions the patient or family has about the rehabilitation process.  This introductory conference is designed to include the patient and their family in the treatment plan process and to set expectations for the balance of the patient’s stay.

Patients are connected to additional support services once they are discharged  through the Liberty family of services so that patients  continue receiving the best care and service through Liberty Home Care and Hospice and Liberty Medical Services.  Click here to learn more about the Liberty family of services.
