Short-Term Rehabilitation
When short-term rehabilitation is recommended following a hospital stay due to an illness or injury, Liberty is the provider of choice in the Carolinas. From the point of admission until you return home, we will be with you on your road to recovery. We offer physical, occupational and speech therapy. We will tailor a plan of care that meets your rehabilitation needs and overall healthcare goals. Our highly skilled, in-house therapy team will diligently work with you to help you regain your function, strength and mobility so you can maximize your quality of life and independence. Our ultimate goal is help you safely return home.
Physical Therapy
Physical therapy helps restore function, improve mobility, relieve pain and prevent or limit permanent disabilities caused by an injury or disease. Our physical therapists work to retrain your muscles to maximize strength, balance, coordination and joint movement. In order to maximize mobility and minimize pain, our physical therapists will implement an exercise plan focused on gait training, transfers, strengthening, balance and improving endurance. They also provide enhanced therapy modalities such as electrical muscle stimulation and diathermy to expedite your recovery. Physical therapy can also assist with wound care, lower body contractures, bracing and prosthetic training for amputees.
Occupational Therapy
Occupational therapy works on the skills used to perform daily tasks so you can increase your independence. Our occupational therapists enable you to do the day-to-day activities that are important to you by overcoming the physical impairments or limitations holding you back. The purpose of occupational therapy is to facilitate your independence by helping you accomplish daily tasks such as bathing, dressing, grooming, eating, toileting and home management. This is done through exercises, education on adaptive equipment, adaptive dressing, feeding aides and energy conservation techniques. Our occupational therapists also focus on wheelchair positioning, upper body contractures and splinting.
Speech Therapy
Speech therapy helps people who have experienced a loss of communication skills, such as speaking or understanding speech, or a loss of cognitive function. Our speech-language pathologists can identify language problems and help improve a person’s ability to express their wants, needs and desires. They also provide specialized services to people who have swallowing difficulties, helping to restore their ability to safely eat and drink.
Enhanced Therapy Services
Many of our locations offer enhanced therapy services. These modalities compliment therapy sessions by reaching deeper muscles and tissues to speed up the recovery process. Diathermy assists with decreasing pain and preps the muscles and tissue for exercise. Electrical stimulation supports improved joint movement and muscle strength. Vital stimulation targets the throat musculature to promote better swallowing. When you are engaged in these enhanced services, you have a more positive experience during your regular therapy sessions.
Outpatient Therapy
Along with inpatient therapy services, we also provide physical, occupational and speech therapy on an outpatient basis. Once you return home, you can continue therapy by visiting one of our locations to receive a level of therapy that is more aligned with being home. You do not need to be a former resident to receive outpatient therapy. You can be admitted to our outpatient therapy program through a referral from your physician.
Skilled Nursing Care
Liberty’s care team of registered nurses, licensed practical nurses and certified nursing assistants provide round-the-clock skilled care to individuals who are recovering from an injury or illness. Our nursing team can provide wound care to those recovering from surgery, trauma, an illness or those with chronic wounds, including pressure ulcers. We also offer IV therapy services to help people with severe infections, nutritional deficiencies or immune diseases. Common uses for IV therapy include administering antibiotics and providing necessary nutrients and rehydration. Our nursing team regularly assists our residents with their goals in support of their physical, occupational and speech therapy sessions. They ensure each resident receives proper education about their medical condition while providing a strong support system throughout their entire recovery.
Assisted Living
Select locations offer assisted living accommodations. Our assisted living apartments and suites are designed for individuals who may need assistance with certain everyday activities, but who don’t require continued medical services. Assisted living at Liberty combines a safe, cohesive community, personalized support services and a devoted care team available 24 hours, bridging the gap between self-reliance and supportive living.
Hospice and Respite Care
For those facing a life-limiting illness, Liberty Hospice provides compassionate care and expert support for a transition marked by dignity and meaning. For primary caregivers, our respite care provides short-term relief so caregivers can take a break from the demands of caring for their loved one.
