Short and Long Term Care Services to Match Your Needs

Liberty Healthcare and Rehabilitation Services understands the importance of providing patients with services that meet their individual goals. Whether in need of short term support following a hospitalization due to illness or injury, or in need of a caring long term residence, Liberty works with the patient and their family to develop a treatment plan focused on maximizing their outcome and quality of life. This individualized treatment plan ensures a patient progresses based on their abilities and is treated with dignity and respect.

Short-Term Rehabilitation

When short-term rehabilitation is recommended following a hospital stay, Liberty is the provider of choice in North Carolina. From the point of admission until discharge back to their prior living setting, we will be with you on your road to recovery. By using resources like state of the art therapy equipment and continuing inclusive interdisciplinary communication our rehabilitation teams are able to work with each patient to provide a safe discharge plan to home.

Physical Therapy

Physical Therapy provides services that help restore function, improve mobility, relieve pain, and prevent or limit permanent disabilities that have resulted from injury or disease. They work to retrain the patient's muscles to maximize strength, balance, coordination, and joint movement. The ultimate goal of physical therapy is to maximize mobility and minimize pain by working on gait training, transfers, strengthening, balance, and exercise. They also provide modalities, such as electrical muscle stimulation and diathermy to expedite recovery. Other special areas of impairment that can be assisted with physical therapy interventions include wound care, lower body contractures and bracing, and prosthetic training for amputees.

Occupational Therapy

Occupational Therapy works with people who have experienced a loss of skills that make every day manageable. They enable people to do the day-to-day activities that are important to them by overcoming or compensating for physical impairments and limitations. The purpose of occupational therapy is to facilitate one’s independence in whatever must be accomplished throughout the day, such as bathing, dressing, grooming, eating, toileting, and home management. This is also done through education on adaptive equipment, such as shower benches, adaptive dressing and feeding aides, and through energy conservation techniques. Other areas of focus for occupational therapists are wheelchair positioning, upper body contractures, and splinting.

Speech Therapy

Speech-Language Pathologists treat people who have experienced a loss of communications skills, such as speaking and understanding speech, or a loss of cognitive function. They can identify language problems and help to improve a patient’s ability to express their wants, needs, and desires. They also provide very specialized services to people who have swallowing difficulties, thus helping to restore their ability to safely eat foods and drink beverages that they enjoy.

Skilled Nursing

Liberty Healthcare and Rehabilitation provides professional medical services twenty four hours a day, seven days a week by members of their care team consisting of Physician services, registered nurses, licensed practical nurses, and certified nursing assistants. Our nursing staff regularly assist our patients with their goals in support of their physical, occupational and speech therapy sessions. By providing education about their medical condition while supporting their recovery, Liberty ensures each patient is more successful upon returning home. Come experience the Liberty difference as we walk with you on your road to recovery.

Wound Care

Our trained staff is available to serve patients who may need wounds treated, including patients who are:

  • Recovering from surgery
  • Recovering from trauma
  • Recovering from illness
  • Recovering from chronic wounds (including pressure ulcers)

IV Therapy

Intravenous therapy (IV therapy) is a type of treatment, in which fluid is delivered into a patient’s veins through an IV drip. This type of therapy is beneficial for patients with severe infections, nutritional deficiencies, or immune diseases. The most common uses for IV therapy include:

  • Administering antibiotics
  • Providing necessary nutrients to patients
  • Assisting with rehydrating patients

Assisted Living

Designed for individuals who may need assistance with certain everyday activities, but don’t require continued medical services. Liberty combines a safe, cohesive community, personalized support services, and devoted care team available 24 hours to administer medications and assisted care, bridging the gap between self-reliance and supportive living.

Hospice & Respite Care

For those facing life-limiting illnesses, hospice care provides compassionate care and expert support for a transition marked by dignity and meaning. For primary caregivers, our respite care provides short-term relief, so primary caregivers can take a break from the demands of caring for their loved one. This is an essential component of a patient's support system, as it will ensure your loved one always receives compassionate, expert care. Whether you need just a few hours or a vacation, respite care will help reduce your stress, restore your energy, and provide a welcome change of routine for your loved one.

Outpatient Therapy

At Liberty Healthcare and Rehabilitation Services, patients have the option to receive physical therapy, occupational therapy, and speech therapy on an outpatient basis at all of our North Carolina locations. By using the same therapist, our outpatient care eases the transition from inpatient to home.

Our facilities also have patients admitted directly to outpatient services through a referral from their personal physician or a physician who provides the first report on workers’ compensation cases. To begin outpatient services, contact your local Liberty facility’s Admissions office with your current insurance information and physician’s order for outpatient services.
